The Current State of PBMs
The healthcare industry is at a pivotal moment as Pharmacy Benefit Managers, known as PBMs, come under increased scrutiny from regulators, lawmakers, and the public. Legislative efforts are also moving forward with rare bipartisan support. Lawmakers are advancing proposals aimed at increasing transparency and eliminating practices that can drive up costs. One key focus is spread pricing, where a PBM charges a health plan more than what is paid to the pharmacy, keeping the difference. Proposed reforms would prohibit this practice and require that manufacturer rebates are passed directly to the employers and patients who ultimately bear the cost of medications.
Several states have already taken action by introducing their own regulations. These include stricter licensing requirements and enhanced transparency measures designed to protect independent pharmacies and ensure consumers have clearer insight into how drug pricing works. These changes are helping to build momentum for broader national reform.
In response to mounting pressure, some PBMs have started to introduce more transparent pricing models. Cost plus approaches and simplified fee structures are emerging as alternatives to traditional systems. While these changes represent progress, many employers and patients believe they are only the beginning. The demand for a more open and accountable system continues to grow.
